At Midwest Monster’s Academy Inc., our Children’s Long-Term Support (CLTS) Support & Services Coordination program is dedicated to empowering children and families by providing personalized guidance, advocacy, and coordinated support services that promote independence, safety, and community inclusion. Our experienced support coordinators work closely with children, parents, caregivers, providers, and community partners to ensure each participant receives services that align with their individual needs, strengths, goals, and family priorities. Our CLTS case management services focus on developing a person-centered approach that places the child and family at the center of planning and decision-making. We assist families with navigating available resources, understanding CLTS-funded supports, connecting with qualified service providers, and accessing community-based opportunities that enhance quality of life. Through ongoing communication and collaboration, our team helps ensure services are delivered effectively and adjusted as needs change.
