Who Is Eligible?
- Are under the age of 22
- Have a documented developmental disability, physical disability, or severe emotional disturbance
- Require long-term support to live safely at home and in the community
- Are eligible for Medicaid
- Reside in Wisconsin
How to Get Started
- Contact your local county CLTS agency to request an intake or referral
- Complete the eligibility determination process
- Select Midwest Monsters Academy Inc. as your CLTS service provider
- Begin services based on your approved service plan
